Customer application forms


Application forms serve many functions beyond the obvious of capturing the customer's information. They are judged by customers that fill them in directly and by sales agents that work on a customer's behalf. Forms need to be well design to cater for rapid and accurate capture of the required information, while ensuring that the compliance requirements of the Customer Identification Program (CIP) are met.


In addition to the application, the customer will be expected to provide proof of identification and address. For business customers the identification requirements are more length. Prior to providing this information, the customer must be informed about what information will be required of them in the form of a visible notice in a branch or on the website.

There are three main application form types that may be used and often an organization will provide more than one.
